You will need the following information on hand in order to complete this application. We encourage you to gather all necessary documents and information before beginning to fill out the application because, after you have submitted it, you will not be able to access the application to make changes. Remember to send your institutional certification officer documents which support your scores for your GPA, your Praxis tests, your clinical experience, and your verbal score. This must be done in order to verify your application.

  • Social security number
  • Current cumulative GPA
  • Secondary level only: current GPA within major
  • Hours of supervised clinical experience (must be at least 400, of which at least 300 hours are directed student teaching)
  • Highest SAT or GRE verbal score or ACT verbal or reading score
    (NOTE: For those students who took the SAT prior to 1995, view this conversion chart, to determine whether your SAT verbal score meets the MNTC requirement. See whether your pre-1995 score converts to a score of 580 or above. When you enter the College Board site, click "SAT Equivalence Tables" in the left hand column. Under SAT Reasoning, click on "Individual Score Equivalents", and you will see the conversion chart. If you scored 500, for instance pre-1995, your score now would be recalculated to a 580, which meets the MNTC criteria for verbal ability.)
